Health Reimbursement Ad

I need a banner that explains the health insurance user how Reimbursement works.

I will explain it here, as I need it to explaing brasilian health Reimbursement, and I don't know if it is equal to other countries:
Who has health insurance havs two ways of selecting his doctor. The first is easier, just select the doctor from the guide book from the health insurance and call for an appointment. Then the payment will be done directly from the health insurance to the doctor. This is very easy to accomplish and most choose this method. Problems of this method: have to wait long for an appointment, consults will be faster, doctor earns less, doctor is staff from the health company so he may be influenced by its needs, like less exams for example.
