Email Sign Up Form (Lightbox)

This is what Tangent Point Media was looking for in their graphic design

We are looking for a 320 X 380 graphic design to be used as a lightbox style email sign up form (if unfamiliar with what a lightbox is, please see attached designs) on our website, https://www.thehealthytrucker.net to capture email sign ups during the holiday season. The theme for this lightbox is “Gifts for Truck Drivers” and we have attached some graphics that have been used in the past to provide inspiration.

We want to keep this text simple and easy to read while using strong graphics to really grab our site visitors’ attention and prompt them to sign up to receive our gift guide emails.
