Poster Design Brief

We have small posters in our dental treatment rooms of the systemic effects of periodontal disease. I want to have a pop up banner created for the waiting room that patients can easily read. I am attaching a copy of the poster we have now (8.5 x 11). The text will stay the same, graphic can change. We will be using the following company to create the pop up poster:
