App Design Brief

We need a simple desktop app (widget style) that will help our agents quickly and efficiently log a details of a contact that they received from client(via email or phone).
App should help our agents to state what happened on the call by enabling them to easily add a list of the services provided.
Some of the data(phone numbers, caller names) will come across automatically from our internal systems e.g. CTI.
