Web Design Brief
Looking for a web page design for second opinion radiology service. Main page will have a link directing user to locate CD on their computer/device, and will take contents of CD and upload to server, which I will specify.
Web page needs a form to enter user's name, contact info, and create a log in for user. User must click to accept terms of service, which will be a paragraph which i will provide. User will log back in, after uploading cd, to find report, which will be able to be viewed once user pays, by paypal. User will need to be able to download report.
There will be a small paragraph explaining what we do: Provide expert second opinions, worldwide. We provide interpretations from radiologists with many years of experience and training from the world's best institutions, including yale and massachusetts general hospital.
