Advertisement Design Brief

We would like an advertisement for a family doctor that mimics a political campaign in the US. I have uploaded a mailer that has gone out. We would like similar information. Our logo, the doctor's photo, and contact page (we want map) are also uploaded.
We would like an advertisement that is for a full page in a magazine. We would also like a smaller version (half page) for other newspaper, etc in color and grayscale/b&w - we would start a new design project with selected designer for half page ad.
