Postcard Design Brief

I am sending out an environmental health survey to all health care providers (MDs, NPs, and DOs) in Indiana to assess their knowledge of environmental health concerns that effect their patients. Based on that assessment, I will then send them targeted free continuing education modules to bolster their knowledge in any weak points. The postcard will have a website link on it that they will go to for the survey (I wish I could email them, but Indiana doesn't collect provider emails, only addresses). I need to somehow pull these providers in to complete my survey and get the free CEs. This is for my doctorate thesis. The survey link is: http://tinyurl.com/envihealth
