1 - The UI styles are basic css with not style added to UI elements like selection boxes, radio buttons and tabs. Do you want the designer to give it new style. ( This is UI part )

2- You mentioned that General Insurance is compulsory, others are optional. Do you want user to clearly understand this with less time ? If the other plans are optional, than you only have one product General Insurance and others can be offered as adds on and make it obvious for users to understand or get it when they look at page. This may need another page where we only show add ones to user where they can select or skip ? ( This is UX Part )
