Business Card Design Brief

I would like a design developed using the logo (number 14951246) previously developed and attached. The information needs to include everything shown on the attached email title block, arranged to suit a business format. I would like a white background, with no other graphics added.
I need an editable version (e.g. MSWord) so I can change the name shown on the business card for others.
