Flyer Design Brief

We need a tri-fold brochure and 1 page flyer/poster for Dr. Jonathan Thomas' dental office located on Maui, Hawaii. We are offering a new "Dental Savings Plan" to patients.
I have included 3 photos, along with the office logo. You do not have to use all photos if you do not want to. We want this to catch the eyes of patients, easy for them to read and understand, and to show them that this is a much better option than dental insurance.
Please use same Hawaiian theme and colors on the office website https://www.smileswithaloha.com .

Please also include the following to promote this new plan:

• Professional Dental Cleanings (2 per benefit year)
• Perio Maintenance Cleanings (3 per benefit year) covered at 80%
• Radiographs as needed (1 set of X-rays per benefit year)

• Additional limited/emergency exam (1 per benefit year)
• Cosmetic consultation
• 15% discount on all other services (including cosmetic procedures)

Annual Fee
$399 first family member/individual (equivalent to $33 per month)
$349 second family member (equivalent to $29 per month)
$249 each additional family member (equivalent to $20 per month)

Provisions of the Dental Savings Plan
• May not be combined with insurance plans, discounts or promotions
• This Dental Savings Plan is not an insurance plan, but a payment arrangement provided by Jonathan Thomas DDS, and is not transferable to any other dental office or dental specialist
• Payment for services are due at the date of service
• This program is for you and is not transferable
• Benefits are provided for one year from the date the plan is purchased
• Must be paid in full when starting the plan
• No refunds will be provided for dues of the plan annual fee under any circumstances, including failure to schedule and maintain appointments
• If Care Credit is used to pay for the program and for services then only a 10% discount will be given
• To receive the full 15% discount on restorative treatment, you must schedule the appointment the day it is recommended. Any failed appointment results in the loss of the 15% discount.
