Graphic Design Brief

I conduct workshops for people who have taken the Gallup StrengthsFinder Assessment which identifies their Top 5 strengths. I need a worksheet to use for sessions on various related topics for participants to write their top three action steps they will take as a result of what they learned. I am not Gallup or an employee of Gallup and so this worksheet is unique to me. That said, I think I'd favor it having a similar look and feel to their materials. Here's their website to get a feel https://www.gallupstrengthscenter.com/. Another organization, who puts on workshops, and who I love the look and feel of their materials is The Strategic Coach.
