I need Dental oriented business cards.
On the front side -
- on the first line, my name:
Dr Jesse Cohen
- on the second line (just underneath):
Doctor of Dental Medicine
- on the third line, or underneath that or in a corner, I would like the address and phone number:

- other than that i would maybe like a logo of a tooth, but i want it to be subtle and very basic!
- I would like the card to be preferably black or dark grey-ish with the writing in white
On the other side -
- 2-3 lines with the possibility to write the next appointment the box with the lines should be white, with the rest of the card being black
