App Design Brief

we have an existing ordering Modal on a trucking management system, but it looks and feels clunky. We would like to modernize the look and feel of it. We need to keep the same input fields, but we are open to any type of reconfiguration or layout changes you can think of to improve the flow and layout.

This design is for a web browser, not mobile app.
