We need a high converting life insurance page created. It will be housed at www.InsuranceQualification.com. We will be driving former life insurance prospects to this page where they will only need to confirm their phone number & email in order to receive an updated quote.

Must have

• 1) Under "InsuranceQualification.com" Please include a "Powered by HGIS" tag and match the colors of this site: http://thehgis.com/life-insurance-services.html to the page.
  
  2) The form should ask for their phone number and include an appointment calendar widget (we'll code the functionality) so they can choose the date/time of their call. It should also have an email confirmation field.
  
  3) the overall message next to the form fields should be "Thanks for visiting Brian (we'll personalize this "firstname" field ), please let us know the best time to schedule a brief call so we can give you an updated, rock bottom pricing quote on a policy.
