My preference is double sided. I just clarified that on the site along with a few other things plus scanned in my current business card as a JPG. I believe the PDF one was not readable.

Is there a logo for ABP Insurance Agency or will it just be typed like on your sample?
Do you requre the ABP Insurance Agency to on both front and back?
