Logo Design Brief

I need to have a logo that an executive had made with AI and wants to use as is created into proper logo files.

Would like to have the logo reproduced in the standard editable vector formats (.PSD, .AI, .EPS) along with standard usable formats (.JPG, .PNG)

Would like to have a few versions of the logo
1) As is with the ICON on the left, company name on the right and slogan at the bottom
2) ICON on the top centered, company name in the middle centered and slogan at the bottom centered
3) Same as 1 & 2 above except WITHOUT the slogan
4) Same as 1 & 2 above except WITHOUT the slogan and in B&W
5) Just the ICON by itself

Lastly would like an official document outlining logo acceptable usage guidelines to include font and color codes
