Business Card Design Brief

I am looking for a designed business tent card for the maintenance department of a hotel. The hotel is a Ramada Plaza, I would like the logo to be on the card, and some verbiage telling the guest that while they were out maintenance came and repaired the issue(s) in the room. I would like the maintenance personelle to be able to check off the common complaints, HVAC, Plumbing, TV/Remote, Lights, other. There are some examples online, I would like it to be polished, and modern looking. My printer is www.gotprint.com, and they only use certain file types.
